Harris County Public Health is working on a new Community Health Improvement Plan. After reviewing these documents, please take a minute to complete the survey below.  The purpose of the survey is to get your opinions about community health issues and our quality of life.  In collaboration with our public health partners, we plan to compile this information and use it as input for the development of our plan.

Thank you for your time and interest in helping us make Harris County a healthier place to live!

Five lucky people who fully complete the survey, and live or work in Harris County will win a $100 gift card! Contact information will be required to qualify. This information will be kept confidential. For questions about the survey, please contact Katie Chennisi at or 713-439-6067.
