2017 Disability Services Needs Assessment

Dear Consumer/Caregiver,

Thank you for participating in the Division of Senior, Disabilities & Veterans Services 2017 Needs Assessment related specifically to Hunterdon County residents with disabilities.   We are seeking input from County residents with disabilities of all ages, and their caregivers, to better identify areas of need.

Responses to these surveys will remain confidential. Your name or any identifying information will not be given to anyone.
