- Anonymous electronic administration of staff survey, including customized poster
- $100 gift card for survey prize drawing winner
- Customized survey results report, including results broken down for each practice site
- 60-minute survey results presentation and facilitated activity at an all-staff meeting
- 90-minute leadership team meeting
- Tailored recommendations, written implementation plan, and resources
Pricing:
Diversity, Equity and Inclusion Assessment & Plan: $4,750 for one practice site + $575 for each additional practice site
Workforce Vitality + Diversity, Equity & Inclusion Assessment & Plan: $4,950 for one practice site + $575 for each additional practice site
If requested, additional implementation support or one-on-one consultation is billed monthly at a rate of $275/hour.

All services are delivered virtually via Zoom. If client requests an in-person meeting, travel time will be compensated according to our travel policy.
