Survey for People at High Risk for Colorectal, Pancreatic or Prostate Cancer Due to an Inherited Mutation

Facing Our Risk of Cancer Empowered (FORCE) is a national nonprofit organization for people and families affected by hereditary cancer. We are conducting a survey to better understand the unmet needs of people who are at high risk for colorectal, pancreatic or prostate cancer due to an inherited mutation. We will use this survey to develop new resources and advocate for better medical options to improve outcomes for our community. We will share the results of this survey with the community, but your individual information will never be shared. You may choose not to include your information, but at the end of the survey you will be given the opportunity to provide your contact information for additional follow up if you choose. This survey is voluntary, you may exit the survey at any time.
