The Montgomery County Board of Developmental Disabilities Services (MCBDDS) would like your opinion regarding the quality and type of services it has provided during the past 12 months to you or your family member.  We would like individuals receiving services to complete this survey as independently as possible, with family members/guardians completing a separate survey.  You can take the survey online at https://www.surveymonkey.com/r/H5F3WPC or download a copy to print  at http://www.mcbdds.org/survey.  

This survey only has 12 questions.  Please answer all that apply to you, and  return it no later than MondayNovember 25, 2019 at 11:59 p.m. 

To return the survey via U.S. Mail, please send it to: MCBDDS Communications Dept., 5450 Salem Avenue, Dayton, OH 45426.  If you receive this survey from a provider, you can return it to your provider, and they will forward it to the Board.  Thank you for making time to share your opinion.
