CONSENT TO PARTICIPATE IN  EDUCATIONAL RESEARCH
Teachers with GUTS Comparison Group Member Consent form

You are being asked to participate in a research study conducted by Eric Klopfer, PhD, from the Scheller Teacher Education Program at the Massachusetts Institute of Technology (M.I.T.). You were selected as a possible comparison group member in this study because you are a member of the TeacherswithGUTS.org online PD network, participated in a Project GUTS professional development workshops and/or online course, or in a Code.org Project GUTS CS in Science Professional Development workshop. You should read the information below, and ask questions about anything you do not understand, before deciding whether or not to participate.

PARTICIPATION AND WITHDRAWAL
Your participation in this study is completely voluntary and you are free to choose whether to be in it or not. If you choose to be in this study, you may subsequently withdraw from it at any time without penalty or consequences of any kind.  The investigator may withdraw you from this research if circumstances arise which warrant doing so. 

PURPOSE OF THE STUDY
The purpose of the study is to investigate methods to promote the uptake of research-based science instruction featuring computer modeling and simulation into regular classroom practice. In particular, we will study teachers' experiences with the Project GUTS resources, PD models, and tools, and assess how well they prepared teachers to implement Project GUTS' Computer Science in Science curriculum.

PROCEDURES
If you volunteer to participate in this study as a "comparison group member", you will be asked to complete a survey about your participation in Project GUTS professional development and implementation of Project GUTS activities, as well a survey of Attitudes, Interests, and Awareness and a survey of Knowledge and Skills that are taught through Project GUTS.

POTENTIAL RISKS AND DISCOMFORTS
Risks to participating in this study are minimal. Taking surveys does not present any known risks to teachers and participation in professional development is an accepted practice.

POTENTIAL BENEFITS
This project addresses the need for a computationally-enabled STEM workforce by equipping teachers with the skills necessary to prepare students for future endeavors as computationally-enabled scientists and citizens, and by investigating the most effective ways to provide this instruction to teachers. 

PAYMENT FOR PARTICIPATION
You will be compensated for participating in the survey with a gift from Project GUTS (valued at $25.00).
 
CONFIDENTIALITY
No personally identifying information will be collected from you.
(We do ask for your name, email and mailing address so we can send you a gift BUT this information will be recorded separately for shipping  purposes and promptly removed from the research study data.)

EMERGENCY CARE AND COMPENSATION FOR INJURY
If you feel you have suffered an injury, which may include emotional trauma, as a result of participating in this study, please contact the person in charge of the study as soon as possible.  In the event you suffer such an injury, M.I.T. may provide itself, or arrange for the provision of, emergency transport or medical treatment, including emergency treatment and follow-up care, as needed, or reimbursement for such medical services.  M.I.T. does not provide any other form of compensation.
