Thank-you for your time in participating this survey regarding whether practice variation impacts burnout in Canadian family physicians.   

This survey is being conducted to evaluate the whether practice variation in both clinical and non-clinical areas is protective against burnout.  Approximately 1,500 family physicians will be offered the opportunity to participate in the survey.

Your responses will be collected through an online survey hosted on SurveyMonkey and should take approximately 5-10 minutes.  Participation in the survey is optional and not time-sensitive but it is important to note that the survey will close 2 months after distribution of this email.

If you participate, you have the option of leaving your e-mail address for the opportunity to win a $50 Amazon gift card. Otherwise there will be no direct benefit to you in completing this survey.  Your email address will not be tied to any of your survey answers.

Your completion of this survey is completely voluntary.   You are not required to provide any personal information such as your name, address or telephone number.  However, some general demographic data will be asked in the survey. You do not have to answer any questions you are not comfortable with. The principle investigator will retain copies of the survey data for five years following dissemination of the study results at which point they will be destroyed confidentially.  The survey data can only securely be accessed by the principle investigator and one co-investigator.  However, following study dissemination results the raw data may be shared for peer verification.

It is not possible to know all of the risks that may happen in a study but the researchers have taken all reasonable safeguards to minimize any known risks to study participants. If there is any breach in confidentiality you will be notified as such and provided with an explanation. It is possible that the survey may uncover burnout.  If this does occur please follow the link here (https://www.cma.ca/physician-wellness-hub/physicians) to find resources for physicians who are experiencing burnout. Please feel free to contact the study principle investigator, anonymously if you so desire, for further resources.
