Firm Sponsorship Evaluation Form Survey

Each time you use the services of a Firm Sponsor, we ask that you complete an evaluation of the Firm’s performance. This evaluation will be held in strict confidence and will be used by NACVA as a tool to monitor and improve the quality of the services offered by our Firm Sponsors. Please take a few minutes to answer the following questions.

1) In evaluating the Firm’s responsiveness to your request and fully assessing the scope of your needs, in your opinion, how quickly and efficiently did the Firm Sponsor respond and size up your needs in the contemplated relationship?
