KEY INSTRUCTIONS FOR THE NAQC FY2012 ANNUAL SURVEY:
IMPORTANT TEST BEFORE YOU BEGIN:
TO AVOID LOSING DATA, PLEASE ENTER YOUR CONTACT INFORMATION, THEN LEAVE THE SURVEY (CLOSE YOUR BROWSER), AND COME BACK USING THE SAME URL. If your contact information is still there, you can leave the survey at any time and come back to complete it later. If it is not there, Survey Monkey does not recognize your computer due to issues with firewalls or cookies. You will need to gather all the information using the PDF or Word versions of the survey, and then enter all the data at one time to avoid losing information and having to re-enter it multiple times.

A. Report on the state of quitlines and trends over time,
B. Compile quitline benchmarks for key metrics, and
C. Continue to make the case for maintaining and increasing quitline funding.
Key data elements will be posted on the quitline profile page for each quitline on NAQC's website (SEE http://www.naquitline.org/?page=mappage)
