Introduction

The Manitoba Advocate for Children and Youth is an independent officer of the Manitoba Legislative Assembly. The Manitoba Advocate investigates and reports on the quality of public services for children, youth and young adults. We make recommendations to improve services, and follow up on the implementation of our recommendations. We also advocate for children, youth, young adults, and their families, including those eligible for disability services.

The Manitoba Advocate wants to hear from the primary caregivers of children and youth under 21, who are living with disabilities in Manitoba that receive services from Children's disAbility Services, or are waiting for services. We also want to hear from primary caregivers whose child was denied services from Children's disAbility Services, or have been unable to access disability services they believe their child requires. 
 
We want to know the issues, barriers, and challenges facing your child and family. The survey questions ask about your child's demographics and disability-related needs, services received, respite, care coordination, daycare/school-experience, and specialized living situations. There are also questions about services during the COVID-19 pandemic lockdown.
 
We will use the information you give us to make a report and recommend changes to improve disability services for children, youth, and young adults in Manitoba.

Important Information:
  • This survey asks questions about your child, your family, and services your child has received.
  • You can stop this survey at any time.
  • You can skip questions if you do not want to answer them.
  • Your answers to this survey are confidential; only the project team will see them.
  • You will not be identified in any reports.
  • Your care providers will not know that you completed the survey.
  • Providing your name and email will be optional upon completion of the survey to receive a copy of the report or participate in interviews.
The survey will take about 5-15 minutes to complete.
