Information Sheet

USC Caruso Department of Otolaryngology – Head and Neck Surgery’s USC Voice Center is conducting a research study. We would like to assess the prevalence of vitamin B12 use amongst singers and voice professionals by doing a nationwide survey.

If you agree to participate in the study, we will ask you to complete an anonymous survey online. There are 20 questions and will ask about your general information, singing history, and vitamin B12 treatment. You can skip or stop answering any questions that makes you feel uneasy.

There are no direct benefits to participation in the survey. However, the knowledge obtained from this study may determine the prevalence and perceived benefit from vitamin B12 treatment amongst singers and voice professionals.

Participation in this study is completely voluntary. There will be no consequences to you if you decide not to participate. You are free to change your mind and stop being in the study at any time.

This study has been granted an exemption by the IRB at the University of Southern California. The IRB is a research review board that reviews and monitors research studies to protect the rights and welfare of research participants. We may publish the information from this study in journals or present it at meetings.
