2025 Community Health Needs Assessment Survey

Every three years, non-profit hospital organizations in the United States, such as FHN, are required by the federal government to complete a Community Health Needs Assessment (CHNA) to identify the healthcare needs of the community. After the CHNA is completed, the organization develops an action plan for the next three years to address the community’s needs. FHN last completed this project in 2022 (available to view at www.fhn.org/CHNA).
