The Illinois Department of Human Services Division of Substance Use Prevention and Recovery (IDHS/SUPR) requires all Substance Use, Treatment, and Recovery Services Block Grant (SUPTRS BG) funded organizations to complete the Annual Certification Plan (ACP) by completing this survey. The Survey is used to collect data not otherwise reported through other existing systems. Data collected is used for federal reporting purposes.
All organizations administering a fixed rate Global contract for recovery homes and/or substance use disorder (SUD) treatment services are required to complete the Survey no later than November 15, 2024. Organizations that administer expense-based grants (e.g., prevention, gambling outreach, Recovery Community Organization/RCO and Recovery Oriented System of Care/ROSC) are not required to complete the Survey.
Every question is mandatory unless there is a skip option. When you have completed the survey, the contact who completes the survey will be sent an email verifying its completion.
