Welcome to Easter Seals Ontario Service Providers' Survey

Easter Seals Ontario is undertaking a review of its services with the goal of developing a plan for the next 3 – 5 years. We need you to help us plan for the future. You have been invited to participate as you are someone who interacts or supports children and youth with disabilities. The survey should take about 10 to 15 minutes to complete. * 2. Have you referred a child, youth or family to Easter Seals Ontario in the past 12 months? Easter Seals provides programs and services for children and youth with physical disabilities. To be registered with Easter Seals, the child must have a permanent disability that restricts their independent mobility and results in the use of Assistive Devices Program (ADP) funded, primary mobility devices such as a wheelchair or walker. The child must also be a legal resident of Ontario, have a valid Ontario Health Card and be under the age of 19 years.
