Welcome to Easter Seals Ontario Family Survey

Easter Seals Ontario is undertaking a review of its services with the goal of developing a plan for the next 3 – 5 years. We need you to help us plan for the future. We welcome the opinions of families who receive Easter Seals Ontario services as well as the opinions of those who do not currently receive services. 
The survey should take about 10 to 15 minutes to complete.

Easter Seals Ontario provides programs and services to  children and youth  who must have a permanent physical disability that restricts their independent mobility and results in the use of an ADP funded, primary mobility device such as a wheelchair or walker. The child must also be a legal resident of Ontario, have a valid Ontario Health Card and be under the age of 19. Do you have a child registered with Easter Seals Ontario?
