Follow-Up Survey for Medicaid Providers Re: EVV Implementation

FABA continues to evaluate the Electronic Visit Verification (EVV) Pilot Project recently implemented by Tellus.  

This survey is to help identify issues with implementing the EVV system.   This only applies to providers in ABA services in Areas 9, 10 and/or 11.
