Parent Survey

Thank you so much for taking this anonymous survey.

This survey is being taken by local parents in conjunction with the statewide Healthy Kids Colorado Survey for 7th-12th graders. We feel it is important that these two surveys occur at the same time as it provides an opportunity for conversation with your child(ren).

Filling out this survey should take  about 10 minutes. All data collected is anonymous. Results will be analyzed by Eagle River Youth Coalition and Communities That Care to provide additional information regarding parent perceptions and behavior in our neighborhoods.

You only need to take this survey once, even if you have more than one child.

It is important to note that we define "youth" as a minor under 21 years of age.

As a thank you for filling out this survey, we would like to donate $10 to the school PTA of your choosing (for the first 250 responses), please only pick one. You will be able to indicate your preference on the last question of the survey.
