This database has been created to collect current information for all EMS agencies that serve the citizens of Ohio.  While the Ohio Department of Safety, Division of EMS has received information via the EMS/Fire Agency Information form, there is additional information needed.



Please note: This database is for agencies that provide prehospital EMS care.  It is not for EMS training facilities/CE sites.
