This survey has been created to gather the feedback about the 'Northern Territory Mental Health and Wellbeing Plan' (The Plan) from people with lived experience of mental distress, family and kin who provide love and support, service providers and the broader community.

The development of The Plan has been lead by AMSANT, NIAA, NT Health and NT PHN. They will use the plan to improve the way mental health services are planned and delivered in the NT over the next five years.

The plan includes a list of priorities that are organised into groups:
1. NT wide priorities
2. Priorities that relate to the five regions of the NT (Central, Barkly, Big Rivers, East Arnhem and Top End).
3. There are a list of activities for each priority. These make up the 'to do list' for the plan

The survey has the same 'structure' as the plan. People with lived experience have worked with NTPHN to create this survey based on the plan, so that the language used in the survey is easier to understand for everyday people. 

The survey is designed so that you can choose which parts of it to complete, and you can exit at any time. You also have the option to provide comments on what can be improved, information about your experience or any other information you think is relevant.
