Run by Creative Lives, the Creative Lives Awards (formerly the Epic Awards) are an annual celebration of voluntary and community-led creativity across the UK and Republic of Ireland.

Please fill in this form to enter the Creative Lives Awards before 11.59pm on Monday 13 May 2024. Make sure you've read our rules and guidelines first!

The application form involves providing some simple contact details and eligibility criteria and we'll ask for a photo. The only thing you might want to think about before starting your application is two 500-word (maximum) questions: an overview of the project and why your project deserves to win a Creative Lives Award.
