Thank you for taking part in the Clergy Wellness Program and for completing your Clergy Health Risk Assessment (CHRA)this past summer.
As you may recall from taking the CHRA prior to attending the Clergy Wellness Retreat in August 2011, The CHRA was initially conducted by the Clergy Wellness Program and its partners at the University of Kansas School of Medicine-Wichita (KUSM-W) in order to help provide you with a comprehensive health profile for you to use at the Clergy Wellness Retreat.
While the initial CHRA proved to be a useful tool in assessing the health and wellness of you and your fellow participants at the outset of the program, it is important to assess any changes in your overall health and wellness that may have occurred since attending the Clergy Retreat last year. Therefore, you are being asked to complete a follow-up CHRA at this time.
As this follow-up assessment is identical to the initial assessment administered last summer, this follow-up CHRA addresses your holistic health, and, as such, this assessment may take you up to 30-45 minutes to complete. While we recognize this time burden, it is important that the information you provided this summer as the foundation for your journey through the Clergy Wellness Project now be compared to a more contemporary health assessment in order to help us assess what impact, if any, participation in the Clergy Wellness Project has had on your overall health and wellness.
Your participation in the Wellness Project and your completion of the CHRA is completely voluntary and greatly appreciated. You may cease completing the CHRA at any time at no penalty to yourself and you may elect not to answer any question that you are uncomfortable answering at no penalty to yourself. The information you provide as part of this follow-up assessment will only be shared with research personnel at KUSM-W as well as your physician. All dats acquired from these follow-up assessment will only be reported in aggregate with no individual identifiers included.
