City EMT Case Manager Job Position |
Position Summary
The Case Manager provides weekly case management services to young adults ages 18 to 24 who are actively participating in City EMT’s EMS Program. The Case Manager will focus on building a partnership with the young adults to create a solid foundation for the transition into adulthood through case management services including: housing stabilization, job coaching, access to supported employment opportunities, and resource procurement and advocacy. Under the supervision of the Program Director, the Case Manager is responsible for engaging participants in their immediate need’s assessment and procurement of resources as a part of a service delivery team. The Case Manager works within the limitations of the organization’s resources and in compliance with grants, contracts, and agency policies and procedures. The ideal candidate will take initiative and be able to quickly and effectively build a rapport with highly vulnerable young adults. They would be comfortable navigating systems, reliable, efficient and committed to the self-determination of marginalized young adults. This person would need to be able to work collegially with colleagues and collaboratively with the supportive service partnerships of City EMT; steadfast in their goal of delivering case management services with impact. Assistance with promoting the program may include participating in pop-up booths and working some evenings and weekends.
