Who may be a Second Victim?

A second victim is a health care team member who is involved in an unanticipated patient event, stressful situation or patient-related injury and who became hurt in the sense that he or she is traumatized by the event.

Second victims often:

• feel personally responsible for the patient outcome.
• feel as though they have failed the patient.
• second-guess their clinical skills and knowledge base. (University of Missouri Health System)

Why are we being asked about this now?

Have you ever experienced a patient care situation where things did not go as expected. One example might be where the patient wasn't suppose to die and you worried about what you might have done differently, or that you missed something? It seemed like you just couldn't get over it. Finally you did and then a few months later, someone asks you about it, or you start talking about it and feel upset all over again. According to safety expert, Dr. Wu you might be a "second victim".

What is the purpose of the survey?

The Hospital would like to do a better job of providing 'emotional first aid' for particularly stressful clinical events that our staff, physicians, students and volunteers may encounter. Some examples may include any traumatic event such as failure of rescue efforts following prolonged intervention, adverse patient outcome related to a medical error, the death of a child, and any other event that is unusually emotionally challenging and stressful in our healthcare environment.

Please take a moment to complete this confidential questionnaire. The results will provide a foundation for decision-making about what the hospital might do to better support you emotionally when you experience a clinically traumatic event while at work.

About the Survey itself

There are 12 questions. This survey is confidential. In other words it can not be traced back to you. When the results are presented, the information will be pooled together, so no one person can be identified.
