You are invited to participate in an online survey conducted by the Schizophrenia Society of Canada (SSC) in partnership with Allergan/AbbVie Pharmaceutical Company. The SSC has asked various organizations to assist in posting the link for the survey. If you are age 18 or over and a resident of Canada, you are eligible. 

Purpose of Survey: As a person with early psychosis or schizophrenia, you can best comment on how your symptoms and the side effects of your medications affect your quality of life and your recovery journey. This survey will gather current information from you on these issues so that we can continue to learn about and understand what helps and what hinders people’s recovery journeys.  This information can then be used to educate decision makers, to create awareness, and to help develop better treatment and service options for people with early psychosis and schizophrenia. 

Participation: The questions in this online survey will focus on your experiences, feelings, and opinions regarding symptoms and side effects of medications. The survey also includes questions about your recovery journey. All questions will give you a choice of responses.  The survey is expected to take 15 to 20 minutes and can be completed at a time convenient to you between July 8 and July 23, 2021.

Benefits: The first 300 respondents to this survey will receive a $10 Tim Hortons gift card, if they so choose.  The survey also may bring some indirect benefits to you as described above, by educating decision makers, creating awareness, and helping improve services and treatments.

Confidentiality and Anonymity:  Internet protocol (IP) addresses or other information which could link your participation to your computer or electronic device will not be used.

Your responses to the survey will be anonymous. No identifying information will be requested. Nothing will link you to your responses.  If you are one of the first 300 respondents and would like to receive a $10 gift card, you will be asked for your email address.

Risks/Voluntary Participation: We do not expect that by participating in this survey you will experience any greater risk than you do in everyday life. It is possible that you may feel that there are some discomforts associated with certain questions.

You are under no obligation to participate in this survey. You are free to stop the survey at any time by closing your browser. Incomplete survey data will not be included in the survey analysis.  Your decision to not participate in this survey will not have any effect on the quality of care and services to which you are entitled or on your relationship with the teams that provide them.

Data Storage: Electronic documents will be kept in a secure, password-protected electronic folder on the personal computers of the consultants working on the survey, accessible only to the consultants.  SurveyMonkey also retains survey data until deleted by the consultant which will occur once analysis and learnings from the data has occurred.

Information about Results: The organizations who posted the survey link will be asked to post a summary of the survey results.  The results will also be posted on the SSC website and other social media and may be presented at educational conferences.
