CQN IIRA Participant Enrollment Survey

Thank you for completing this important enrollment survey for the AAP's Chapter Quality Network (CQN) Improving Immunization Rates for Adolescents Project. All participants will be asked to provide individual contact information and to complete a participation and data agreement. These questions should take approximately 10-15 minutes to complete. Participating physicians and practice team physician leaders will be prompted to respond to additional questions, which should take 5 minutes (for participating physicians) to 15 minutes (for practice team physician leaders) to complete.

This survey includes a participation agreement (for all participants) and questions regarding current practices (for physicians). Therefore, we ask each participant to submit his/her own response and refrain from completing the survey for other team members.
