Academy Prior Approval Requests for Continuing Education

This form has been created to allow Staff, DPGs, and MIGs the opportunity to submit upcoming live and self-directed programs/events to CDR for their review and approval of CPEUs. All submissions must now meet the new CDR CPEU Prior Approval Program policies and activity types. Please familiarize yourself with the policies listed here to ensure a seamless experience.
PLEASE NOTE: For each CPE activity, CDR has mandated specific survey questions be asked, which can be found on page 38 here.

Guidelines
  • All submissions must be received 7-8 weeks prior to event to allow for processing prior to event.
  • The contact information provided on this form will be used by the Lifelong Learning staff team for additional follow up and distribution of the approval certificate for use at the meeting. Additional information to support the approval of the program may be requested by the Lifelong Learning team.
