Dear Community Member,
The Central New Hampshire Health Partnership wants to learn about the health of your community and we want to hear from YOU.
“Your community” can mean different things to different people. For the purpose of this survey, your community can be as big as all of Central New Hampshire or as small as your neighborhood. Answer the survey questions by thinking about the area you see as “your community.”
Please take 5-10 minutes to give us your thoughts and opinions. Survey results will help us shape our community health programs.
The survey is completely anonymous. You will not be asked for your name or contact information.
