I agree to abide by the conditions set out in the Surf Life Saving Sydney Section Policy, further Guidelines for Selection in the 2018 Lifesaving Sydney Interbranch Team, and the Surf Life Saving Sydney Sports Athletes Agreement.

I agree if I am selected in the 2018 Surf Life Saving Sydney Interbranch Team to pay levy of $250.00 (Clubs will be invoiced for the amount). All Levies are to be paid to the Surf Life Saving Office no later 5pm on Monday 3rd December 2018. I further agree that if the levy is not paid by the due time that Team Management has reserved the right to remove me from the Team.
