The form consists of 13 questions and should take less than 20 minutes to complete. RIS accepts nominations from technology vendors, consultants, retailers and partners. If you have sensitive information included in the nomination form (such as internal metrics) that you would prefer not be published, but which you would like us to consider as part of the application, please note this in the text box where the information appears or where asked to do so specifically in the application. Information not labeled in this way will be considered by RIS’ editors to be publishable.

The individual being nominated is not required to have the CIO title but must be the highest ranking IT executive in the organization.
