CHKD Community Health Needs Assessment Survey

Dear community member,

Children's Hospital of The King's Daughters (CHKD) is doing a Community Health Needs Assessment to understand the healthcare needs of the communities that we serve. As part of this, we are collaborating with Bon Secours Hampton Roads to collect input from a variety of sources. We will then analyze this data to identify our region's most pressing healthcare needs. 

 

Please share your thoughts on healthcare issues facing our community. The analysis of the survey results will be shared with the public, but no data collected will be used to identify you.
