2) Minimum of two (2) years' experience in a long term care facility in Arkansas (Administrator, DON and Nurse of the Year categories require (4) year's experience in a long term care facility in Arkansas)
3) Minimum of one (1) year experience in the nominating facility (Administrator, DON and Nurse of the Year categories require (2) year's experience within the nominating facility)

In any facility where the administrator (within the last 3 years) has had any substandard quality of care, Immediate Jeopardy, Type A Citations, or is a Special Focus Facility, the nomination must disclose and explain the citation. Although disclosure will not automatically disqualify the nominee, it is important the judges and the association be aware of these citations.

A minimum of 3 letters of recommendation are required with each nomination.
You can attach more than 3 (from residents, family members, other staff etc.) if you like, but it is not required.

The 3 required letters must be from:
1. The facility Administrator or Owner/Operator
2. DON or Department Head
3. Co-worker, consultant or another department head 

The letters should include:
  • How is the nominee an exceptional caregiver/team member
  • Describe how your nominee interacts and supports other departments and staff members
  • Illustrate examples of the nominee's initiative and commitment to their job/role
  • Describe the impact the nominee has on the working environment, fellow staff and residents 
  • Describe instances where the nominee went above and beyond the call of duty 
  • Tell the awards committee why your nominee deserves this award
