We are asking for your help to evaluate our services and programs. We want to know what you think about how we are doing and what we could do better. We want your honest opinions.
This survey is completely anonymous and confidential – your name does not appear anywhere and staff will not see your individual responses. Whether you choose to answer the survey will not affect the services you receive here. A summary of the results of all survey responses will be made available in July 2022. We will also keep you updated on any changes made from what you suggest.
