The application has two parts:
1) REQUIRED:Complete this online form and submit. The form will request your contact information, eligibility requirements, essay, letter of recommendation, and acknowledgment/signature.
2) OPTIONAL:Mail your letter of recommendation in an envelope with the author's signature across the seal to one of the following addresses:

Alternately, you may choose in Part D of the online form to use one or both of your letters of recommendation from your MBA admissions application.
Applicants must submit all materials by 5:00 p.m. Eastern Time on March 27, 2012. If you are concerned about hard-copy materials reaching us on time, please email CASE Assistant Director Ruth Tolman at rt10@duke.edu to let her know that the materials are on their way (we will accept recommendation letters after the deadline). You may also email Ruth if you have any questions about the application itself.
