Intake Questionnaire |
This questionnaire will help us select participants to take part in a phone-based interview. The interview will be 30 minutes to an hour long, and an honorarium of $100 will be provided to interviewees in recognition of their contribution to the study.
This questionnaire may take between 5 and 10 minutes to complete. Participation in the questionnaire is voluntary.
If you do not submit your responses at the end, any information provided to that point will be entirely deleted.
The data collected through this questionnaire will be used to find interview participants. You will be asked to provide your name and your preferred method of contact. This is done in order to be able to reach you to schedule your interview if you are selected.
