Application Instructions

This application has 3 sections.  All sections must be completed in order to submit.  You must upload a transcript from the most recently completed semester/trimester of school.
CAHMCP Student Eligibility Criteria
You must be:
  • A URiM student
  • A graduate of an Illinois high school; proof of eligibility will be required. 
  • Currently enrolled as a second-year or a third-year student at an accredited 4-year college/university and pursuing a bachelor of science degree in a premedical program, health professions or other science, technology, engineering, or math (STEM) degree.
  • Able to demonstrate a current GPA of 3.5 or higher; GPAs between 2.75 and 3.4 require a student essay indicating why you should receive special consideration
  • Able to provide proof of a passing grade in undergraduate biology (2 semesters), chemistry (3 semesters, 1 semester ideally organic chemistry), 1 semester of physics preferred, this will be verified via transcript
  • Able to provide proof of a  ”B” or higher in pre-calculus or calculus
  • Prepared to submit a personal statement
  • Agreeable to taking a baseline assessment of math skills upon enrollment in the program

Ideal applicants can:
  • Demonstrate financial need; or
  • Confirm they are first-generation college students

Preference will be given to students that commit to attend all events and participate in all activities.  

In addition, applicants who have previously completed requirements for other CPASS funded programs (ie Mini-Medical School, SMASH, INSPIRE, or MAPSCORPS) and comply with all of the requirements above, will receive priority status.
