Introduction to the survey

My name is Elena Savoia. I am a researcher at the Harvard University T.H. Chan School of Public Health and would like to invite you to take part in a survey about the internet. You may take the survey if you are 14 years or older.

We are asking you to complete a 10-minute survey on your thoughts and opinions about the internet. You will be asked to take the same survey after two months from today. You will be entered in a raffle to win one of FIFTY $50 Amazon gift cards.

This survey is voluntary and your survey answers will be kept confidential. Your teachers, school administrators, and parents will not know your answers. None of the your survey answers will be reported in any way that may reveal your name.

You can stop the survey at any point. If you are unsure about taking the survey, you may talk to your parents. The survey will ask you to enter your school ID, which will be kept confidential. Your school ID will be used only to link your two surveys and also enter you in the raffle.
