Welcome to the WCHN Excellence in Care Matters Consultation Survey

The Excellence in Care Matters survey will be available from Monday, 31 July to Friday, 11 August 2017.

Your experience and ideas are really important to us. The purpose of this survey is to help us understand your views on what excellent care at the Women’s and Children’s Health Network means to you.

This will help us identify what elements make up the concept “excellent care”; what the Women’s and Children’s Health Network  is doing well in terms of delivering excellent care for every person, every time; what the priorities for improvement should be; and what learning and development needs are critical to achieving success.

The feedback will help us develop the Women’s and Children’s Health Network Safety and Quality Framework and Strategy. Your responses will directly contribute to how excellent care is defined, how it can be supported and how it will be implemented throughout WCHN.

When reflecting on the questions posed in the survey, consider what are the critical components of care you or your loved ones would want to receive if they required care at the Women’s and Children’s Health Network.

Your individual responses are completely anonymous and treated with strict confidentiality. No identifying data will be recorded.

We are using SurveyMonkey to collect the data. You can access SurveyMonkey’s privacy policy at https://www.surveymonkey.com/mp/policy/privacy-policy/

Your responses can be long, short, single words or sentences. There are no right or wrong answers.

Thank you for participating in the Excellence in Care Matters survey.
