2023 HDC Board of Directors Candidate Nomination/Application Form

Introduction

Thank you for your interest in the HDC Board Nomination process for candidates to join the Board of Directors in 2024. The following pages will ask for the information required to submit a nomination/application. For open-ended questions, a couple paragraphs or a series of bullet points are equally acceptable.

You can press the "Prev" button at the bottom of each page to go back and edit your answers. Once you hit "Done" on the last page, you will not be able to go back and change your responses.

Please note, in order to submit a nomination, the candidate must be:

Aware of and consent to being nominated.
Willing and able to abide by and comply with both:
- HDC Board Member Commitments and Code of Ethics
- HDC Board Conflict of Interest Statement
