The information collected from this application is for the exclusive use by SPQA for the purpose of selecting the Board of Examiners for the 2019 cycle. All information is kept confidential and is not shared with any other organization. Note: Questions preceded by an asterisk require responses.
Before completing this application, please take the time to review the 2019 Examiner Guide found on the SPQA website www.spqa-va.org. Especially, review the examiner time commitment for the various SPQA programs; Recognition Award, Discovery and Ones to Watch. Also, please review the Virginia SPQA Code of Ethical Conduct for Examiners found on the last page of the 2019 Examiner Guide. You will be asked in this application if you have read and understand the examiner time commitment and the Code of Ethical Conduct for Examiners.
