You are invited to participate in a survey that is part of a study examining a program at your school. This study is being conducted by American Institutes for Research (AIR). If you agree to take this survey, it will ask you for your opinions about your school, classes, teachers, other students, and yourself. The only right answers to these questions are your honest opinions. It will take about 20 minutes to complete this survey.

This survey is voluntary. You can decide not to participate at any time. If you do not want to answer a specific question, you may skip it, but we hope you will answer as many questions as you can. This survey is confidential; we will not tell anyone, including your parents, principal, and teachers, what you say in this survey.

Although there are no direct benefits to taking this survey, your responses will help us to understand how a program at your school impacts students.
