South London Care Home Pioneer Programme Cohort 6 2024-2025 Application Form

The South London Care Home Pioneer Programme is open to care home managers, deputies, and senior nurses working in the following care settings in south London: older person’s residential homes, older person’s nursing homes, learning disability and mental health care settings, and supported living settings.

The Health Innovation Network (HIN) South London and My Home Life England are delighted to announce the sixth South London Care Home Pioneer Programme – a leadership support and professional development programme for social care Managers, Deputies, and Nurses to advance their skills, facilitate personal growth and enable them to effectively manage the complex everyday issues that impact on the quality of their service.

The programme begins with four face-to-face workshops, followed by a series of six action learning sets, some delivered virtually and some face-to-face (see the programme of dates in the flyer). The celebration day on 5th March 2025 showcases the achievements of the Pioneers to the funders and other stakeholders.

Action learning recognises that individuals learn best when they learn with and from each other, by working on real problems and drawing on their own experiences. Three of the six action learning sets will provide additional support for the quality improvement project.

We are delighted to offer this programme free of charge to South London care home leaders, generously supported by South West London Integrated Care Board and The Health Foundation.

Being part of the programme involves:
  • Commitment from you and your organisation to attend all workshops and action learning sets throughout the year. Although offered free to your care home, your place costs £3,000 to funders. Attendance of over 75% is required by funders to complete the programme. By submitting an application form, you are confirming you have booked out all relevant dates in your diary.
  • Delivery of a quality improvement project including a poster to describe your work.

To apply for a place on this programme, please fully complete this form by Wednesday 15th May 2024, 9am. Successful applicants will be notified by Friday 17th May 2024. If you have any questions, please send to our programme email address, hin.pioneerprogramme@nhs.net
