We want all our clients to give feedback on our services. We do this in our Annual Snapshot Survey. We hope this makes services better for you, and all our clients. Doing this also shows the people who fund our services if we are making a difference.

This feedback is anonymous. That means we won’t find out who said what. If you give us comments then we might use the comments when we talk about our services. We talk about taking care of your information in our Privacy and Confidentiality policy. It’s available from your practitioner, at reception or at www.rasa.org.au/privacy

After completing the survey, you may choose to provide your contact details separately. Eight people will be picked at random to receive a $50 Coles Myer voucher.
