Beta blocker increases the chances of stroke and death if started preoperatively in a patient without strong indication like angina or after MI.

Routinely start patient on Beta blocker preoperatively regardless of condition and type of surgery.

Consider starting Beta blocker as soon as possible before surgery if indicated.
