Thank you for participating in the Oregon Robotics Tournament and Outreach Program and FIRST® LEGO® League.

To register for a FIRST LEGO League Qualifying Tournament in Oregon, you will use this form to:
- identify the team and coaches, including contact information
- select three different tournaments for your 1st, 2nd and 3rd choices
- request special accommodations for team members
- provide demographic information about your team members (this information is only used in aggregate) 
- affirm that all adults will abide by FIRST LEGO League values, policies, procedures, and standards
- select your payment method

Information from these forms is used by the ORTOP Planning Committee to assign teams to Qualifying Tournaments and to compile statistical information for grant applications and reporting.

This application will close on October 12th at 5:00pm.

Qualifying tournament assignments will be posted at www.ortop.org no later than November 7, 2016

Payment must be received before your team is considered fully registered. Payments are due by October 31, 2015.
