You are being asked to participate in a voluntary anonymous research study led by Cannabis Center of Excellence, INC and UMass Dartmouth. This study is approved by extension by the UMass Dartmouth Institutional Review Board. 

Your participation is entirely voluntary.  You can refuse to participate without penalty or loss of benefits to which you are otherwise entitled.  You can stop your participation at any time and your refusal will not impact current or future relationships with UMass Dartmouth or CCOE. To discontinue participation you should close your internet browser window. Please print this consent form for your records.

Total estimated time to participate in the study is less than 20 minutes.

Risks of being in the study: The risks of this study are no greater than those associated with everyday life.

Benefits of being in the study: Your participation helps to build the body of knowledge regarding COVID-19's impact on medical cannabis patients and adult use consumers. Your input will help develop citizen and community driven, clinical, public health, policy, and industry recommendations and education materials to protect medical cannabis patients and adult-use consumers amid the COVID-19 pandemic.

Confidentiality and Privacy Protections 
We have taken all reasonable measures to protect your identity and responses.  The questions in this survey do not ask you to reveal any personally identifying information, the transmitted data are SSL encrypted and stored in a secure password protected database, and IP addresses are not collected.  However, email and the internet are not 100% secure, so it is also suggested that you clear the computer’s cache and browser history to protect your privacy after completing the survey.  The data resulting from your participation may be made available to other researchers in the future for research purposes not detailed within this consent form. In these cases, the data will contain no identifying information that could associate you with it, or with your participation in any study.

The records of this study will be stored securely and kept confidential.  Authorized persons from The University of Massachusetts Dartmouth, members of the Institutional Review Board, and Cannabis Center of Excellence, INC have the legal right to review your research records and will protect the confidentiality of those records to the extent permitted by law.  All publications will exclude any information that will make it possible to identify you as a subject.
