We are inviting you to participate in a brief research study through the American Society of Clinical Oncology (ASCO). The purpose of this study is to learn more about patients’ experiences with cancer care. More specifically, we hope to learn how topics like diet, exercise and weight management are incorporated into cancer care. There will be no direct benefits to you for participating in this survey, but information from surveys like this one will help improve how cancer care is delivered.  The survey should take no more than 10 minutes to complete.  

If you agree to participate in the study, we will keep your survey data completely confidential. No one will have access to your data except for the research study staff. The completed surveys will not contain any information that could identify you and will be stored in a secure electronic database. All of the information collected will be destroyed after publication of resulting papers. You will not be identified in any papers that are published from this survey. There are no anticipated risks from participating in this study.  

You do not have to take part in this study. Your decision to participate or not is completely voluntary. Thank you for your help with this important research.
