Overview

In response to the COVID-19/Coronavirus pandemic, Heart of Florida United Way has established a fund to support Central Florida residents whose financial stability is being impacted.

The focus of the fund is to support the ALICE (Asset Limited, Income Constrained, Employed) population experiencing hardship due to decreased hours or unpaid leave.

This form will ask you to share information about your household and your current financial hardship. Once you have submitted this form, Heart of Florida United Way's case manager will contact you in approximately 10 business days. When contacted, you will be asked to submit your required documentation by email or fax.

* 1. Do you live in Orange, Osceola, or Seminole county?

The fund provided by Heart of Florida United Way is only for residents of Orange, Osceola or Seminole County. If you live outside of this area, please call 2-1-1 for assistance.

* 2. Have you had a loss of income or unexpected expense as a direct result of COVID-19?

Heart of Florida United Way's ALICE Fund for COVID-19/Coronavirus is to help those households who have lost income as a result of the Coronavirus pandemic.

* 3. Is your rent/ mortgage and/ or utility bill due in the next 10 days?

If your rent/mortgage and/or utilities are not due in 10 days, your case may be prioritized accordingly.

* 4. Do you have proof of income for all adults?

Proof of income for all adults in the household must be submitted in order to qualify for case management and potential emergency financial assistance.

* 5. Do you have a valid picture ID for everyone in the household who is 18 or older?

You must be able to provide copies of the picture ID for all adults in the household. When requested, these copies must be submitted by email or fax.

* 6. Do you have social security cards for all household members or an original legal document showing the social security number (SSN)?

* 7. Do you have the birth certificates of all the children in the household, or another legal document with a date of birth (passport, immunization records, school records, DCF records)?

When requested, you must be able to provide copies of the birth certificates (or another legal document with a date of birth) by email or fax.

* 8. Do you have the bills and expenses for the household for the past month?

You will be required to discuss your household's total income and expenses with Heart of Florida United Way's case manager. You will be required to provide proof of income and proof of expenses to include bank statements, pay stubs, and bills.

* 9. Are the bills and/or lease/mortgage statement in your name or another adult household member’s name?

In order to apply for emergency financial assistance, the rent/mortgage and/or utility bills for which you are requesting assistance must be in the name of the person providing income and expense documentation.

* 10. Do I have your permission to pass your contact information onto a case manager for further assessment?

Answering yes will give Heart of Florida United Way's case manager permission to contact you and discuss your household's situation.
