IDHS/SUPR FY 23 Annual Certification Plan

The Illinois Department of Human Services, Division of Substance Use Prevention and Recovery (IDHS/SUPR) requires all Substance Abuse Block Grant (SABG) funded providers to complete The Annual Certification Plan, or ACP. This survey is used to collect data not otherwise collected for purposes of federal reporting. 

All funded providers are required  to complete this survey no later than December 19th, 2022. Every question is mandatory. When you have completed the survey, you will be sent an email verifying your completion. Your contract manager will be notified and review your responses. They will notify you if the department needs more information.
