Describe survey and determine if listing company should complete survey.
This free directory is for companies providing medical specialty services (such as radiology, rheumatology, neurology, psychiatry) and ancillary services (such as patient education and language interpretation) through telemedicine to healthcare providers such as hospitals, clinics, nursing homes, private practices and urgent care centers. If your company is providing ONLY medical services directly to consumers, or if you provide only technical services such as platforms, software or connections, please don't fill out the survey. A good way to tell if your company should be listed is to ask, "are the healthcare professionals who provide the consultations employed by or contracted with my company?" and "do the services we offer touch the patient?" If so, your company should be listed. If not, this directory is probably not appropriate for your company. The Arizona Telemedicine Program reserves the right not to publish responses from companies that are not providing medical or ancillary health services or that do not have a website, or for any other reason. Information from this survey will be posted on an online Telemedicine & Telehealth Service Provider Directory aimed at hospital administrators and other healthcare provider decision-makers. This brief survey contains about 30 questions. Questions marked with an asterisk must be completed before advancing to the next part of the survey. Detailed instructions for completing the survey are here. Survey revised February 2022
