Introduction

Thank you for taking the time to complete this ACORNS Quarterly Report (originally due January 29, 2016, with an extension through February 12, 2016). Your responses will help guide our assistance for your ACORNS program's chronic disease prevention and management strategies.
This Quarterly Report focuses on the following strategies, which have the potential to bring about sustainable Policy, Systems, and Environmental (PSE) changes:
  • Preventing Commercial Tobacco Use and Exposure (ex. commercial tobacco-free areas, smoke-free areas, promoting the increased use of tobacco cessation resources such as California Smokers’ Helpline, etc.);
  • Healthy Eating (ex. a healthy food policy that sets food service guidelines/healthy food standards, providing food vouchers, requiring restaurants to provide nutritional information of menu items, etc.);
  • Increasing Access to Local Foods (ex. community gardens and farmers’ markets);
  • Increasing Access to Exercise/Physical Activity (ex. exercise/physical activity policy, community parks and recreation areas, and a Complete Streets Plan to increase access to safe walking and biking paths, routes, and/or trails); and
  • Increasing Community-Clinical Linkages (ex. increasing community-clinical linkages to support chronic disease prevention and self-management, increasing access to Community Health Representatives who link patients to community resources such as Talking Circles and lactation support, increasing use of team-based care strategies, etc.)
